Community Health Programs
Foot Care Clinics: We offer specialized foot and nail care services monthly for Elders and individuals with diabetes. For more information or to schedule an appointment, please contact your local Community Health Nurse.
Transportation Assistance: Community Health provides transportation to medical appointments when staff are available. For scheduling, please contact our Home Care Scheduler.
Medication Pick-Up: We offer medication pick-up services for all Community Health clients to ensure access to necessary medications.
Community Health Nursing
The Ho-Chunk Nation Community Health Nursing Program operates across six office locations: Black River Falls, La Crosse, Nekoosa, Baraboo, Wittenberg, and Tomah. Our comprehensive nursing services include:
- Skilled nursing care
- Immunizations
- Diabetic foot care
- Health screenings
- Caregiver program for assistance with daily living activities, transportation, and respite services
